Chronic Disease Management Patient Empowerment Programme
The Programme is for members suffering from diabetes mellitus, hypertension, low back pain or degenerative knee pain, and those who have had a hip fracture, post-acute myocardial infarction or stroke.
Throughout the Programme, the Centre’s team of medical professionals will provide education, encouragement and advice on lifestyle choices, as well as mental and social support, to help participants manage their condition. The team includes registered nurses, physiotherapist, occupational therapist, dietician, pharmacist and social workers.
The Programme is not open to the public. If you have any of the above chronic diseases, please contact our Centre to arrange a consultation and enrolment.
